1.     A statement of information on family status is issued by the Ministry of Justice of the Republic of Armenia.

The statement of information is signed by the head of the Civil Status Acts Registration Agency and sealed with the trilingual seal of the Agency.

2.     The Ministry of Justice of the Republic of Armenia issues the statement of information:

-       in Armenian (Form No 1 with original seal and signature), (Form No 1 with electronic seal and signature)

-       Armenian-English (Form No 2 with original seal and signature) (Form No 2 with electronic seal and signature)

-       Armenian-Russian (Form No 2 with original seal and signature) (Form No 2 with electronic seal and signature)

3.     The statements of information on family status issued by the Ministry of Justice of the Republic of Armenia (Form No 1, Form No 2) are prepared through the
e-governance system and issued in paper version or electronic version, upon the desire of the applicant.

An application for receipt of a statement of information on family status may be submitted to the Ministry of Justice of the Republic of Armenia, as well as via www.e-services.moj.am.

When submitting an application to obtain a statement of information through the www.e-services.moj.am electronic platform, the applicant is provided with the opportunity to have the statement of information certified with an apostille. If the applicant wishes to obtain the statement of information certified with an apostille, he or she must make an indication in the relevant field when filling out the application through the aforementioned platform.

The statement of information certified with an apostille is issued within two working days. This two days period is calculated after the general time limits prescribed for issuance of a statement of information and the time limits reduced through the paid services, if there are such time limits. A state duty of AMD 5 000 is levied for certifying the statement of information with an apostille. A state duty of AMD 13 000 (AMD 5 000 and AMD 8 000) is levied for obtaining a statement of information certified with an apostille within one working day.

When the application is submitted to the Ministry of Justice of the Republic of Armenia, the statement of information is issued to the applicant by hand and in paper version.

When the application is submitted through the www.e-servicesmoj.am electronic platform, the statement of information may be issued to the applicant by hand and in paper version or sent electronically to the e-mail address of the applicant. The applicant selects the form and option for receipt of the statement of information while submitting the application electronically.

4.     The paper version of the statement of information is approved with the original seal of the Civil Status Acts Registration Agency of the Ministry of Justice of the Republic of Armenia and the signature of the head (or acting head) of the Civil Status Acts Registration Agency.

The electronic version of the statement of information is approved with the electronic seal of the Civil Status Acts Registration Agency of the Ministry of Justice of the Republic of Armenia and with the electronic signature of the head (or acting head) of the Civil Status Acts Registration Agency.

The aforementioned two versions of the statement of information on family status have equal legal force (*Notice: statements of information with electronic signature and seal are issued starting from 4 June 2020).

One statement of information is issued per application. The version of the statement of information issued is selected by the applicant. One statement of information is issued under the same number. A duplicate of the statement of information is not provided. If the statement of information is lost, a new application shall be submitted as prescribed by the legislation.

The following may apply to the Ministry of Justice of the Republic of Armenia to receive a statement of information:

-       the person regarding whom the statement of information is issued or

-       the person authorised by the applicant.

When the authorised person submits an application, he or she needs to submit the following to the Ministry of Justice of the Republic of Armenia:

-       copy of the identification document of the authoriser

-       identification document of the authorised person

-       power of attorney and the statement made by the authoriser /exemplary form of statement and power of attorney/.

5.     The statement of information is issued on the 7th working day following submission of the application.

The statement of information on citizens of the Republic of Armenia who do not have (have not had) a permanent place of residence in the Republic of Armenia and are record-registered in the State Population Register of the Republic of Armenia, is issued on the 10th working day.

6.     The following state duties and extra-budgetary payments are levied for issuance of the statement of information:

a.     in case of issuing the statement of information in Armenian on the 7th working day — AMD 1 000

b.     in case of issuing, in Armenian, the statement of information on citizens of the Republic of Armenia who do not have (have not had) a permanent place of residence in the Republic of Armenia and are record-registered in the State Population Register of the Republic of Armenia — AMD 16 000

In case of issuing the statement of information in the Armenian-English or Armenian-Russian language pairs, another AMD 5000 is levied in addition to the fees specified in the aforementioned points “a” and “b”.

The total of 7 (or 10) working days envisaged for issuance of the statement of information may be reduced upon the desire of the applicant in case of making the relevant payments according to days:

-       in the course of two working days — AMD 12 000

-       on the third working day — AMD 8 000

-       on the fifth working day — AMD 5000

The payments envisaged for reducing the time limit for issuing the statement of information do not include the payments envisaged for issuance of the statement of information specified in points “a” and “b” and in the Armenian-English (or Armenian-Russian) version.

A 16-digit number plate and a quick response code (QR code), generated through the e-governance system, are inserted on the statements of information on family status. The authenticity of statements of information is verified by entering the number fixed on them into the official website for verification of authenticity of official documents running at the Ministry of Justice of the Republic of Armenia (www.e-verify.am) or via the QR code (*Notice: The statement of information is prepared via the Electronic Governance System with the 16-digit number plate and QR code starting from 20 March 2020).

Applications are accepted at the reception of the Ministry of Justice of the Republic of Armenia every working day from 10:30 to 12:00 and from 15:00 to 16:30. Address: 3/8 Vazgen Sargsyan Street, Yerevan.

In addition, the statements of information on family status are also issued by the consulates of the Republic of Armenia in foreign states.

*Notice: The following sample forms have been provided before March 2020:

-       Armenian (Form No 1)

-       Armenian-English (Form No 2) or /exemplary form of statement and power of attorney/

-       Armenian-Russian (Form No 2) or /exemplary form of statement and power of attorney/
