What are the grounds for state registration of death?

Death of a person is subject to registration without time limitation, for which the following are required:

Grounds for state registration of death shall be the following:
(1) prescribed form of a document certifying the death, issued by a medical organisation or a doctor;
(2) a court judgment, having entered into legal force, on confirming the death of the person or recognising the person as dead;
(3) the notarial act on the legal fact confirmed by the notary public as prescribed by the Law "On notaries".
What documents are submitted for state registration of death?

The application shall be submitted to the service centre or diplomatic service agency.

The following documents are attached to the application:
(1) prescribed form of a document certifying the death, issued by a medical organisation or a doctor;
(2) identification document of the applicant;
(3) identification document or data contained therein of the deceased person;
(4) a court judgment, having entered into legal force, on confirming the death of the person or recognising the person as dead;
(5) notarial act on the legal fact confirmed by the notary public as prescribed by the Law "On notaries".

The application shall include the following information:
(1) name, father's name, surname, nationality of the deceased (nationality is mentioned in the identification documents of the deceased where available), place and time of birth, gender, citizenship, last place of residence of the deceased, marital status, place and time of death;
(2) data of identification documents of the deceased;
(3) data of documents confirming the fact of death;
(4) name, father's name, surname and passport data of the person having submitted the application;
(5) other required information.

Any person may apply for state registration of death.
What is the procedure for the state registration of death?

State registration of death is carried out on the same working day when the application and the required documents are submitted.

The procedure and terms for reporting on state registration of death to other bodies or organisations, as well as the list of those bodies or organisations shall be established by the Minister.
